Club member.
Club Membership is only granted to established clients of Trees Chiropractic Clinic, (in good financial standing).

Benefits
- Up to 3 adjustments per month per adult family member
- Additional visits beyond 3 can be purchased for $30 (vs. $50)
- Children under 18 years of age are adjusted for FREE, up to 3 visits per child per month
- Minimum 20% discount on nutritional supplements, pillows and topical analgesics.
If your child or adult family member is not currently a client of Trees Chiropractic, they will be charged a small, one-time
examination fee of $25 - (a 50% savings on what non-club members pay).

There is no long-term obligation and you can cancel at any time.
I reserve the right to cancel this club at any time for any reason. If I do cancel the club, I WILL fulfill my obligation
to you on your paid membership to the end of the month.
Your insurance will not be billed for this membership as health insurance companies do not cover wellness or prevention
or anything related to them.
